Required Registration Documents

  •  Printed version of child’s last report card (School year 2024-2025)
  • New Participants must provide an original Birth Certificate with seal (we will need to keep original until the AYF league reviews and then we will return)
Important! In order for your child to be eligible to participate in the 2025 season, ALL participants require a current physical (dated January 1, 2025 or later). If your child is scheduled to get the annual physical after July 31, 2025, all we need is the med clearance form to be dated, signed and stamped by your physician anytime before July 31, 2025.


All forms/paperwork must be completed and passed in by 7/14 in order to participate in the 2025 season.
